Why medical payment method matters
Visitors who are not enrolled in Japan’s public health-insurance system may be billed the full cost of care. Some hospitals or clinics require payment before discharge, and not every provider accepts every card. A high headline coverage limit is less useful if you cannot access assistance when treatment is needed.
Ask whether the insurer offers cashless service or direct billing with suitable facilities in Japan. This normally requires calling the insurer before treatment except in a life-threatening emergency. If the policy is reimbursement-only, confirm how you would fund a large upfront payment and which documents must be retained.

Credit-card insurance: verify activation
Some cards include travel insurance, but conditions differ. Coverage may activate automatically, or only after paying a qualifying fare with the card. Dependents may have different limits, and trip length can be capped. Ask the issuer for the certificate and full wording, then compare medical and evacuation limits with a standalone policy.
Buying after arrival
JNTO notes that some insurance can be purchased after arrival in Japan. Eligibility windows, age limits, waiting periods, and exclusions vary, so buying before departure usually gives more time to compare terms and may include cancellation cover.
